FORT WAYNE, Ind. (WOWO): If you’re a current or former Lutheran Health Network patient, keep an eye on your mailbox.

Lutheran’s parent company, Community Health Systems, is sending out notices this week of a pending settlement they’ve reached over a class-action lawsuit filed because of a 2014 data breach.

CHS reported that 4.5-million patients’ personal information was exposed, including names, birth dates, and social security numbers, by cybercriminals.

The Journal Gazette reports that anyone who gets a notice was probably affected, and you’ll still have to file a claim by August 1st to be eligible for either up to $250 for those whose info was leaked, or up to $5,000 for those who became victims of identity fraud.

Despite the settlement, CHS maintains it did nothing wrong.
